• ☐ Letter of Authorization (LOA) — Lithuania-specific LOA template, must be downloaded and completed
• ☐ Latest invoice — Current billing documentation from the existing provider
• ☐ VAT/TAX ID — Required if the number belongs to a company
• ☐ Personal ID code (asmens kodas) — Required if the number belongs to an individual

Key Definitions

• Asmens kodas: Lithuania's personal identification code, a unique numeric identifier assigned to individuals. Used instead of a VAT/TAX ID when the number is owned by a person rather than a business.
